Best guessBlizzard has announced only Spring 2029 (BlizzCon, Sep 12, 2026) and told DayOne it is very confident of hitting it. We count to the end of that window on Diablo 4's pattern, which launched on a Tuesday in early June (Jun 6, 2023, from 4 p.m. PT the day before): Tuesday, Jun 5, 2029. Blizzard

Story and Setting

Diablo 5's premise: the heroes of the past failed, Diablo conquered Sanctuary and rules from the throne of Westmarch, and the player is the Heir to Westmarch, about a hundred years after Diablo IV and seven years into Diablo's reign.

Diablo 5 is the story of a Sanctuary Diablo has already conquered: about a hundred years after Diablo IV and seven years into his reign, he rules from the throne of Westmarch, and you play the Heir to Westmarch, who can enter his Terror Realm and survive. The central premise, in Blizzard's words, is that "the heroes of the past have failed, Sanctuary has fallen, and Diablo has won." Diablo has conquered Sanctuary as "a stepping stone to invading the Heavens" and rules from the throne of Westmarch. Asked on the Forging the Next Era panel what story the team is telling, the answer was short: "Sanctuary has been destroyed and Diablo has won."

Why Diablo has won

Narrative director Jennifer Hepler explained that once Diablo 5 was going to bring Diablo back, he "couldn't just be a final boss": he had to be "the force that's shaping this game from its opening moments". So this time "other heroes rose up to try to stop Diablo, and they failed", and players live in "the world that all of the other Diablo games were trying to prevent".

Executive producer Matt Zitterman said the team was "hungry" to see Diablo represented again, since Mephisto and Lilith had been given "great stories and lots of depth", and "Diablo deserves that and more". As another panellist joked, nobody wanted to tell Diablo "that his brother gets his own game and he doesn't".

The team considered making Diablo a morally gray villain, but kept arriving at the same conclusion: "Diablo is Diablo. He's the Lord of Terror." He is "not redeemable", so the question is "not is he right, but what happens now that he's won?"

Diablo "has a lot more to say in this game": he is "a fully developed character" who shapes the story from the beginning, with lines such as "Not even death can save you from me."

A world worth saving

Sanctuary is dark, but the team wants it to be a place worth defending. Defeating Mephisto worked, the panel said: "There was a golden age of Sanctuary", and players could make it that again by defeating Diablo. The story pulls back the curtain on the Eternal Conflict, showing Sanctuary caught between Diablo's aggression and "the almost equally damaging indifference of the heavens", and it lets the player find an ally among the angels and "turn the powers of hell back against itself".

Westmarch and the Heir

The player is the Heir to Westmarch. The game returns to the Western Continent, which has become "essentially greater Westmarch": all of it overrun and turned into "an outpost of hell". Hardly any humans have not bowed or joined the cults; some powerful ones have been corrupted into Dread Commanders with "the power of the dukes of hell", and anyone who resists is put into a soul cage or pulled into the Terror Realm, "Diablo's own corner of hell".

The story begins when the Heir finds that they can enter the Terror Realm and survive, and learns there of other powers the team is not ready to talk about yet.

Inspired by the bonds players formed with Lilith and Mephisto in Diablo IV's Lord of Hatred, the Heir has "a unique bond with Diablo", explained over the course of the game. It gives insight into him, but also "a unique vulnerability" to what he can do, so the game becomes a question of whether you can resist his influence and turn his powers against him.

When it is set

The team told IGN that Diablo deserves "a full campaign and full story", and that it has "jumped a hundred years ahead" so that Diablo IV has room to continue its own story.

Within that, "it's been seven years since he's conquered Sanctuary", and Diablo and his Dread Commanders are reshaping the land. The team calls it "terror forming"; see Procedural World.

The team added that Diablo IV's content "is all in the east", while Diablo 5 is back in Westmarch, "which everyone is familiar with from the first three games", and a largely unexplored future history of the Western Continent.

Survivors

There are "no normal civilians left". The people you meet travel with you in the Caravan, and their stories replace the quest givers of earlier games. Blizzard's first two named are a former Sister of the Sightless Eye and a stranded Treasure Goblin.

What is still unknown

Blizzard has shown the premise, the hero and the villain, but not the plot. As of Oct 18, 2026 it has not said which other characters from earlier games return, who the angel the Heir may ally with is, what the Heir's other powers are (the panel said the team is not ready to talk about them), or how the story ends. Nothing more has been revealed since the Forging the Next Era panel at BlizzCon 2026 in September; this page grows only with Blizzard's own reveals (Release and Development).

Westmarch itself is familiar ground. It is the kingdom to the west of Khanduras, the land of Tristram, and its capital was the battleground of Diablo III's expansion Reaper of Souls (2014), where the Reapers of Malthael, the former Archangel of Wisdom, overran Westmarch City. Diablo 5 returns to a Westmarch that a different enemy has taken, a century and more later.
